Should Children Take Multivitamins?

Many parents worry that their children may not be getting enough nutrients from food alone, but they're also not sure if they should be giving them a multivitamin.

Unfortunately...there is no consensus. Experts tend to disagree about vitamin recommendations for children who don't display signs of deficiency. The naysayers claim that children don't need large amounts of vitamins and minerals, and that even picky eaters should be able to get enough from food. On the other side are those who think of a basic children's multivitamin as an "insurance policy" that can fill in the gaps of a not-so-great diet.

In my office, we approach this case-by-case. If it's clear that a kid is not eating a well-rounded diet, we may recommend a children's "multi." If we feel a parent is unduly concerned, we try to reassure them, but will also give them the option of an "insurance policy" multi.

Is there a downside to the "insurance policy" approach?

It's probably fine. My only concern is that some foods are already overly fortified with vitamins and minerals (specifically some breakfast cereals and bars). In other words, even kids who don't take a multivitamin often get one without even knowing it.

Two nutrients you don't want to overdo:

1. Vitamin A (specifically from retinol, palmitate, or acetate; if it comes from beta carotene, it's fine). Chronic intake of excessive amounts through fortified food and supplements can cause big problems.

2. To a lesser degree, zinc is also a concern. Too much zinc over prolonged periods of time can depress your immune system and lead to a copper deficiency.

So check the fortified foods your child eats. If they provide 100% vitamin A and/or 100% zinc, and you're not willing to lose that fortified food, do not add a multivitamin to the mix. On the other hand, if your child does not eat a lot of fortified foods (or the fortified foods do not contain an abundance of straight vitamin A and zinc), you can feel free to go ahead and add a standard multi.

If you decide to give your child a multivitamin, choose a brand that's formulated specifically for children and that provides about 100% of the RDA for all of the vitamins and minerals listed (always avoid megadoses unless prescribed by your pediatrician).

Coffee Protects Against Blinding Eyelid Disorder

TUESDAY, June 19 (HealthDay News) -- A daily dose of java may help prevent a form of severe eyelid spasm that can cause a person to become "blind," a new study suggests.
In people with primary late onset blepharospasm, the eyelid muscles contract uncontrollably and cause involuntary blinking. In extreme cases, people are unable to prevent their eyelids from clamping shut and, despite having healthy eyes, they become functionally blind, according to background information in the study.
The new study included 166 Italian patients with primary late onset blepharospasm and two control groups -- 187 people who were relatives of the patients and 228 people with another kind of facial spasm disorder -- who were asked about their coffee consumption.
A team at the University of Bari found that just one to two cups of coffee a day helped protect against late onset blepharospasm and that increased coffee consumption also delayed onset of the condition -- 1.7 years for each additional cup of coffee per day.
The study authors noted that previous research has suggested that caffeine may help prevent Parkinson's disease by blocking adenosine receptors, and the same mechanism may play a role in coffee's protective effect against primary late onset blepharospasm.
"Considering that the caffeine content of a cup of Italian coffee (60-120 milligrams) is similar to the average content of a cup of American coffee (95-125 milligrams), the protective effect on the development of blepharospasm may be exerted at caffeine doses greater than 120-240 milligrams, comparable with the caffeine doses suggested to be protective against Parkinson's disease," the study authors wrote.
Their findings were published online June 19 in the Journal of Neurology, Neurosurgery and Psychiatry.
Previous studies have suggested that smoking helps protect against blepharospasm, but the authors said they found no evidence to support that theory.

Folic Acid Supplements Cut Stroke Risk

FRIDAY, June 1 (HealthDay News) -- Folic acid supplementation may reduce the risk of stroke by 18 percent or more, but it's not clear whether it boosts outcomes for other cardiovascular conditions, researchers say.
A study published last year in the journal Circulation found that there was a significant drop in stroke death rates in Canada and the United States after both countries mandated folic acid fortification of cereals and breads in 1998.
For the new research, a U.S. team reviewed eight studies of folic acid supplementation, which lowers concentrations of homocysteine in the blood. High homocysteine levels are believed to increase the risk of stroke, cardiovascular disease, and deep vein thrombosis.
Professor Xiaobin Wang, of Northwestern University Feinberg School of Medicine in Chicago, and colleagues found that folic acid supplementation reduced the risk of stroke by an average of 18 percent.
Even greater risk reduction were noted when treatment lasted more than 36 months (29 percent lower risk); when homocysteine levels were reduced by more than 20 percent (23 percent lower risk); or if a patient had no previous history of stroke (25 percent lower risk).
In regions that did not already have supplementation via fortified foods, the introduction of folic acid supplementation reduced stroke risk by 25 percent.
"Our meta-analysis provides coherent evidence that folic acid supplementation can reduce the risk of stroke in primary prevention," the study authors wrote.
The review appears in the June 2 issue of The Lancet medical journal.
"Although this meta-analysis helps clarify answers to some questions about the role of homocysteine lowering in CVD (cardiovascular disease) prevention, ongoing randomized trials are needed before we can conclude that the benefit of continued use of previously deemed 'safe' vitamin supplements outweighs the risk of other adverse CVD outcomes," Dr. Cynthia Carlsson, of the University of Wisconsin School of Medicine and Public Health, wrote in an accompanying comment article.
